Forms, Formats and Figures — the training image library
Rajasthan ULB General Election 2026
Purpose. Every form, annexure and machine diagram the training material refers to, held here as an image of the official page, so trainers show the real format rather than a description of it.
Nothing here is redrawn or reconstructed. Each image is a direct render of the official PDF page, with its source and page number recorded. Where a form's content is summarised in a handbook, the summary carries the same citation as the image.

🚨 Cataloguing error worth knowing. Order 1374 dated 04.02.2026 is titled "सां.आ. 155/2026 — उपाध्यक्षीय पदों के परिणाम के सम्बन्ध में" on the SEC website and in the Compendium index (item 66). Its operative text — verified in two independent copies (the SEC page PDF and the Compendium's own copy at printed p 273) — is about सदस्य पदों (member posts), under Rule 72 read with Rule 68, requiring the RO to send result documents to the Commission within three days of declaration. Circular [2292 पैरा 28] cites it correctly for member results. The title is wrong in both catalogues; the text governs.

⚠️ These figures are MPSV. The Margdarsika states that the pink paper sealing procedure is identical for MPSV and M3A / MK-5, and that MPSV figures are used there purely for illustration [MG Ch.4-A, printed p 33]. For M3A / MK-5 carrying cases and unit layout, use the guide's own figures at [MG Ch.4-A, printed p 28] (FIG. 1 and FIG. 1(A)).
6. How to use these in a session
- Never describe a form you can show. Project the annexure image, then walk the columns.
- परिशिष्ट-15 (Check Memo) and परिशिष्ट-14 (task outline) are the two sheets a Presiding Officer should physically hold on poll day — print and laminate them.
- परिशिष्ट-8 is a filled specimen of Form 14-ग. Use it to teach the paper seal account at item 9 and the total at item 5 — the two entries the entire counting-day tamper check depends on [2292 पैरा 16(1), पैरा 20(1)].
- For the pink paper seal and ABCD strip, show
page_37→page_41in order; the five strip steps are the most-failed practical task. - परिशिष्ट-15A (envelope list) belongs at the close-of-poll station, next to the sealing table.
7. Still to be identified

- Letter 705 Annexures 1–4 (SDMM pink paper seal, envelope, two stickers) — referenced in SDMM / DMM Sealing After Counting — the procedure §4 but their format pages have not been located in the Compendium.
- The statutory printed forms (14-A, 14-C blank, 15, 15A, 16, 16A–16D, 19, 20) as they appear in the Rules' schedule — the ULB Manual holds them; not yet extracted.
