What this module is for. Forms, Formats and Figures — the training image library is an image library (which form is on which page image). This file is different — it says who fills it · when · in what situation · and what can go wrong.

🎯 Four questions for every form

1. Who fills it? · 2. When? · 3. What situation triggers it? · 4. Where does it go? Ask exactly these four in class. A trainee who can answer all four knows that form.


§0 — 🚨 Three rules first, then the tables

# Rule
1 "प्ररूप" (Form) comes from the Rules; "परिशिष्ट" (Annexure) comes from the Margdarsika or an order. Two separate series — Annexure-8 ≠ Form 8. Annexure-8 is in fact the filled specimen of Form 14-ग
2 Never say a number on its own. "Bring Annexure-1" is the most dangerous sentence in this election — see §4
3 🚨 The rule reference printed on a form is a "label", not an instruction. When a rule changes the label stays behind (e.g. "rule 45-A" printed at item 3 of Form 14-ग — that rule is now deleted)

🚨 The attestation block needs the attesting officer's signature, designation AND seal — all three. That is the commonest cause of rejection. A tick or a cross is also valid — do not reject merely because no stamp was used (SO 157 Explanation I · order 1418 item 12(d))

§4 — 🚨 Traps — lookalike names, changed foundations

4.1 One name, three different documents

4.4 🆕 In the Compendium, but it does not reach an EVM poll

✅ So what happens in that situation on an EVM?

If the elector refuses to sign or thumb-mark the voter register 14-क → they are not permitted to vote, and they are counted at item 4 of Form 14-ग (rule 35-A). 🔴 No ballot paper is cancelled, no endorsement is written, no rule 37(7) cover is used.

⚠️ I am not saying order 295 is wrong or repealed. It is the procedure for a ballot-paper poll and sits in the book as such. Balotra's member poll is on EVMs, so the PRO will never use it.

4.3 ⚠️ Not yet positively identified

🚨 This "not yet known" list is deliberate. Leaving a name blank is better than filling it by guesswork.
