Worked specimen forms — for the Presiding Officer
Rajasthan ULB General Election 2026 · Balotra · one imaginary booth, every form reconciling to the others
⛔ First — this entire file is DUMMY
Form structure, item names, rule references ✅ official — taken from the Margdarsika's annexures and Form 14-ग, which I read first-hand Names, figures, machine numbers, candidates 🔴 all invented — for teaching only 🚨 Never file a copy of this in any real record. It is a classroom blackboard, not a document. ✅ The genuine blank forms are in
training/forms/— print from there.
🎯 What this file is for
A Presiding Officer's commonest mistake is arithmetic — items 2, 3, 4 and 5 failing to reconcile; the paper-seal subtraction; the diary not agreeing with Form 14-ग. So everything here belongs to one booth, and every form carries the same figures. What is in 14-ग is in the diary, and in the mock-poll certificate.
How to run it in class: hand out the blank forms → show this worked copy → then change the figures and have them fill it themselves → make them do the item-6 reconciliation aloud.
📋 Our imaginary booth — carried through every form
| Field | Value (invented) |
|---|---|
| Body | Nagar Parishad Balotra |
| Ward number | 27 |
| Polling station no. and name | 84 — Govt. Upper Primary School, New Town |
| Part number of the electoral roll | 3 |
| Date of poll | 11.09.2026 (Balotra is Phase 2 — this date is real) |
| Control Unit (CU) | RJSCU 084213 |
| Ballot Unit (BU) | RJSBU 084577 |
| Presiding Officer | (name) — invented |
⚠️ Balotra uses MPSV/MPMV, so the machine comes with an SDMM, not a DMM. That difference matters for counting and custody → BALOTRA §2
1. 🔴 Form 14-ग · Part 1 — Account of votes recorded (filled)
(rules 49-ग and 63-क(2) · the format at Margdarsika Annexure-8)
| Item | Description | Filled value |
|---|---|---|
| 1 | Total number of electors assigned to the polling station | 1142 |
| 2 | Total number of voters entered in the voter register (Form 14-क) | 869 |
| 3 | 🚨 Number of voters deciding not to record their votes ("No Vote") | 4 |
| 4 | Number of voters not permitted to vote under rule 35-A or 37-A | 2 |
| 5 | Total number of votes recorded as per the voting machine | 863 |
| 6 | Does item 5 = item 2 − item 3 − item 4 ? | ✅ Yes, it tallies → 869 − 4 − 2 = 863 |
| 7 | Number of voters issued tendered ballot papers under rule 44-A | 3 |
Item 8 — account of tendered ballot papers
| Sub-item | Serial numbers | Count |
|---|---|---|
| (a) received for use | 0201 to 0220 | 20 |
| (b) issued to electors | 0201 to 0203 | 3 |
| (c) not used and returned | 0204 to 0220 | 17 |
✅ Check: 3 + 17 = 20 = (a) ✓ · and the count at (b) matches item 7 = 3 ✓
Item 9 — account of paper seals
| Sub-item | Description | Value |
|---|---|---|
| 1 | Serial numbers of paper seals supplied | 004821 to 004824 |
| 2 | Total number of seals supplied | 4 |
| 3 | Number of paper seals used | 1 (004821) |
| 4 | Number of unused paper seals returned to the RO | 3 |
| 5 | Serial numbers of seals destroyed, if any | Nil |
🚨🚨 Item 9(4) — there is a printing error here, be careful
The form prints the bracket "(subtract item 2 from item 3)" — that is inverted. It would give 1 − 4 = −3, which is impossible.
✅ The correct calculation: item 2 − item 3 = 4 − 1 = 3
This defect is already recorded in our material → DLMT correction sheet, item 1-ढ Tell the PRO before they leave, or they will follow the printed instruction and fill it wrongly.
At the foot
Date 11.09.2026 · Place Balotra · signatures of polling agents (1 to 6) · signature of the Presiding Officer · polling station number 84
⚠️ The Margdarsika's own specimen slips here — the head says station "16" while the foot says "polling station number 15". Write the same number in both places.
2. 🔴 Form 14-ग · Part 2 — Result of counting (filled)
| Sl. | Candidate's name (invented) | Number of votes recorded |
|---|---|---|
| 1 | A | 268 |
| 2 | B | 191 |
| 3 | C | 142 |
| 4 | D | 97 |
| 5 | E | 88 |
| 6 | F | 61 |
| — | 🚨 NOTA (none of the above) | 16 |
| Total | 6 candidates + NOTA | 863 |
✅ Check: 268+191+142+97+88+61+16 = 863 = item 5 of Part 1 ✓
🚨 Never leave the NOTA cell blank
In the Margdarsika's printed specimen the NOTA row exists but carries no figure. On an EVM, NOTA is a button — it will always have a count, even if it is zero. If it is zero, write "0" — do not leave it empty.
3. Annexure-19 — Mock Poll Certificate (filled)
| Field | Value |
|---|---|
| Polling station | 84, Ward 27, N.P. Balotra |
| Time of mock poll | 07:00 to 07:20 |
| Polling agents present | 4 (for A, B, C, E) |
| Total votes cast in the mock poll | 14 |
| Minimum per candidate | at least 1 each, NOTA included ✅ |
| 🚨 After the mock poll | CLEAR pressed and the result zeroised ✅ |
| Total after CLEAR | 0 (shown to the agents) ✅ |
| Agents' signatures | 4 |
🚨 This is the single biggest lapse — forgetting to press CLEAR
If the mock-poll votes stay in the machine they are added to the real votes. ✅ Sequence: mock poll → CLEAR → press Total and show 0 → agents sign → then seal the machine. ➡ If CLEAR was not pressed and polling has begun — inform the RO at once; do not decide alone.
4. Annexure-7 — Presiding Officer's Diary (selected items, filled)
The full 30-item list → EVM/hi module 10, §5 Given here are the items that must agree with Form 14-ग — that reconciliation is the lesson.

✅ The diary's golden rule
item 11(2) = 14-ग item 2 · item 12 total = 14-ग item 5 · item 16 = 14-ग item 7 · item 6 = 14-ग item 9(3) · item 21 total = 14-ग item 5 🚨 If even one of these fails to match, some entry is wrong — fix it before leaving the booth.
🚨 Two obligations attached to the diary
1. If there is not enough space against an item, attach a plain sheet to the diary. 2. Do not write "nothing" at items 28 and 30 if something did happen — this diary is later the only contemporaneous evidence in a re-poll or in court.
4-A. 🔴🔴 An order about the diary that was in no module — order 858 dt 4.10.99
Where it came from: the Commission Compendium, item 40, printed p. 152 — page read first-hand. Subject: "Municipal elections — the Presiding Officers Diary"
| # | What the order says |
|---|---|
| 1 | 🔴🔴 The diary shall on no account be kept in the strong room with the sealed ballot box — it stays in the custody of the RO or an ARO |
| 2 | The RO shall study every Presiding Officer diary — or, where absolutely necessary, an ARO nominated by him |
| 3 | 🔴 Where a report for adjournment of the poll or a fresh poll is to be sent to the Commission under rules 53 and 55, the diary must necessarily be studied first |
| 4 | 🚨 "During training, Presiding Officers shall be explained in detail how to fill the diary properly" |
🎯 Direction 4 falls directly on the DLMT
Explaining how to fill the diary in training is the order own requirement — it is not optional. The worked specimen at §4 above exists for exactly that.
🚨 Take direction 1 seriously
The diary does not go to the strong room. If it is sealed away with the machine it cannot be read when a re-poll question arises — and under direction 3 the report cannot be sent without reading it. ➡ This is the commonest slip when making up the envelopes — the diary goes into the non-statutory envelope, and that envelope goes to the RO/ARO, not to the strong room.
Source: Order 858 dated 4.10.99 — Compendium item 40, printed p. 152. The Commission reprinted it in its own 2026 Compendium — that is the basis for treating it as in force.
5. Annexure-13 — Challenge fee receipt (filled)
| Field | Value |
|---|---|
| Receipt number | 02 |
| Amount received | ₹2/- |
| From | (name of the challenger — invented) |
| Against which elector | Roll part 3, serial no. 412 |
| Rule | 🔴 rule 43(5) (printed on the counterfoil) |
| Date | 11.09.2026 |
| Signature of the Presiding Officer | ✔ |
✅ When this sum is forfeited — this is now settled → CHALLENGED_VOTE_en
The Margdarsika says "challenge found correct → forfeit"; rule 43(5) says "frivolous or not in good faith → forfeit, in any other case → return". In the commonest situation the two are opposites. ➡ Until the DEO answers in writing, do not state either rule as final in class — show both texts → DEO question B1
6. Annexure-11 — List of electors who made age declarations (filled)
| Sl. | Roll part · serial no. | Elector's name (invented) | Declaration taken |
|---|---|---|---|
| 1 | Part 3 · 118 | — | ✔ |
| 2 | Part 3 · 655 | — | ✔ |
✅ Total 2 — this must agree with diary item 17(a) ✓
7. Annexure-15A — List of envelopes (filled, in brief)
| Envelope | Contents | At this booth |
|---|---|---|

| Non-statutory | diary · mock-poll certificate · challenge receipt · age declarations · companions' declarations | ✔ |
| Unused | unused paper seals 3 · unused tendered ballot papers 17 | ✔ |
🚨 Both the 3 unused seals and the 17 unused tendered papers must agree with 14-ग ✓ (item 9(4) = 3 · item 8(c) = 0204–0220 = 17)
8. ✅ One-page final reconciliation — before leaving the booth
| # | Check | In this dummy |
|---|---|---|

| 2 | Part 2 total = item 5 | 863 ✓ |
| 3 | NOTA cell filled (write 0 if zero) | 16 ✓ |
| 4 | Tendered: item 7 = count at item 8(b) | 3 = 3 ✓ |
